Admissions: Where to Begin?

Finding the right "fit", and doing it soon, is a considerable challenge in meeting the needs of a brain-impaired individual.  The first step is to contact us to discuss your circumstances and explore if we're a good match for your needs.  We may then schedule a clinical interview and conduct an assessment, which would generate a Plan of Care for your consideration.  We will spend the time to get to know the client, their history and their support network.  This time is also an opportunity for you to get to know us.  Our success, both the clients and the programs, is predicated on mutual goals and our ability to work with each other.  Each potential candidate applying to C.C.I. must demonstrate:

    1.  Medical stability (this does not negate ongoing medical need!)

2.  The potential to benefit from our program model

3.   A willingness to be free from substance abuse

4.  An identified funding source

We will work closely with our clients' families and their funders to assure that we are all in agreement on goals and the program's direction.  We will ensure that funds are utilized in the most efficient way to maximize service availability while assuring long-term financial stability and resources for our clients.

We welcome inquiries and invite you to visit us.  If you would like to make a referral or need additional information, please contact us.
