The Center for Community Independence serves individuals at various levels of cognitive and behavioral functioning.  Those who need 24-hour supervision and those who are employed, living relatively independent lives, may all benefit from our individual client-specific treatment approach.  Our one-to-one staffing allows each client to participate in an individualized program independent of any other client program at the Center.  Our model has made it possible for us to establish individual programs throughout the country.  Our client specific individual programs are separated into two basic categories:  Transitional Living and Supported Living.

  •     The Transitional Living program is designed for individuals who are in need of intense and often comprehensive rehabilitation services.  The program is designed as a vehicle to transition our clients along the continuum from the hospital to the community and to facilitate a greater level of functional independence.  It is relatively short-term in length, with specific goals, timelines, and therapies tailored to client needs and transition plans established upon admission. 

  •     The Supported Living program is for individuals who have demonstrated improvement after their injury but continue to need support to live a relatively independent life and to make further but perhaps slower gains.  It is generally long-term in length.  In this program, all clients are engaged in meaningful, community-based activities throughout the week, including employment when appropriate, with an emphasis on their safety and quality of life.

  • In addition to the transitional and supported living programs, we offer home-based programs, day and respite programs, assessments, and comprehensive evaluations.  Our one-to-one staffing and client-specific programs allow for the flexibility and creativity to address and effectively meet individual needs and circumstances.  Our services include:
